Before You Inject: Why Dental Cleanings, Vaccinations, and Flying All Matter for Filler Safety

But as seasoned cosmetic medical professionals know, the patient's immediate timeline: what they did last week, and what they plan to do tomorrow: matters just as much as where we place the needle.

Today, let’s look at three specific lifestyle and health events that should trigger a strict temporal pause before you pick up a syringe: dental cleanings, vaccinations, and upcoming flights.

1. The Dental Clean: Transient Bacteremia and Biofilms

We’ve all seen it: a patient books in for mid-cheek filler, completely forgetting they had a dental scale and clean three days ago, or worse, scheduled for one tomorrow.

Why does this matter?

A routine dental cleaning or periodontal procedure triggers transient bacteremia: a temporary dumping of oral flora and bacteria into the bloodstream. If circulating bacteria encounter freshly injected dermal filler, they can seed the implant site, colonize existing sub-clinical biofilms, and trigger delayed inflammatory nodules or deep infections weeks down the track.

The Clinical Rule:

• The 2-Week Gap: We suggest enforcing a strict minimum 2-week buffer either side of a dental clean or dental work. If there was an active dental infection or extraction, stretch that to 4 weeks.

• Intake Integration: Make "Have you had, or do you have scheduled, any dental work or cleaning in the last or next 14 days?" a mandatory tick-box on your consultation intake form.

2. Vaccinations: The Immune Cascade

Over the last few years, we’ve all navigated post-vaccination filler swelling. Whether it’s the annual flu shot, a COVID-19 booster, or the shingles vaccine, any systemic immune challenge revs up the body's immune system.

When the immune system goes into overdrive, it recognizes hyaluronic acid filler as a foreign body. The resulting immune cascade can cause acute, distressing swelling, tenderness, and localized lumpiness at previous injection sites: even months or years after the initial treatment.

The Clinical Rule:

• Timing is Everything: Just like dental work, require a 2-week buffer before and after any vaccination.

• Informed Consent: Ensure patients understand that getting a booster right before or after cosmetic injections isn't dangerous per se, but it significantly elevates the risk of temporary inflammatory flare-ups that will require clinical intervention.

3. Flying: Altitude, Cabin Pressure, and 35,000 Feet

This is one of the most common conversational hurdles in clinic. A patient wants lip filler on a Thursday afternoon before flying out to Bali or Europe on Friday morning.

Here’s the hard truth: if a patient suffers a vascular occlusion mid-flight, you cannot manage it at 35,000 feet. There is no emergency access to hyaluronidase on a commercial flight, and cabin pressure fluctuations can exacerbate acute tissue edema and compromise perfusion further.

The Clinical Rule:

• The 24-48 Hour Rule: Avoid injecting any patient who is flying within 24 to 48 hours. Give yourself: and them: enough time in the post-procedure window to monitor for early signs of vascular compromise.

• Document and Defend: If a patient pushes back because of travel plans, document the conversation explicitly in your notes. Record that you advised them of the risks associated with flying post-procedure and that they understand your duty of care overrides their holiday itinerary.
